Privacy Act Agreement for Request of Motor Vehicle Records

Purpose: The services offered on the OCIO website are for businesses, organizations, and governmental agencies to obtain multiple driving record histories on individuals pertaining to their business or other daily operations.

A fee of $10.30 is charged for each driver’s license look-up unless it is made by state, county, or city officials, court officials, public transit officials or other officials of a political subdivision of the state or a non-profit charitable organization described in section 501(c)(3) of the Internal Revenue Code. Non-profit, charitable organizations must be otherwise eligible for access. This does not include any credit or debit card processing fees that may apply.


Instructions:

  • This Privacy Act Agreement for Request of Motor Vehicle Records must be completed and approved before a Requestor can obtain personal information or highly restricted information from the Iowa Department of Transportation.
    Only Requestors who meet the criteria outlined in Part B are eligible to obtain such information from the Iowa Department of Transportation. This Agreement must be completed with all required attachment before the Iowa Department of Transportation will consider a request for motor vehicle records.
  • Requestor must attach a legible photocopy of his or her driver's license or non-driver identification card and proof of authority to act on behalf of a government agency or private entity.
  • Requestor must print his or her full name on each page of this agreement.
  • Requestor will be notified—typically via email—whether the request is approved, denied, or missing required information.
